To the victor go the spoils. So if such an event happened your likely not to read about it. The victor writes the history books in other words.
You could argue the point that world war 1 wiped out alot of empires, The Austro Hungarian, and Ottoman Turkey. The fall of Rome. Empires usually decline over time.

Probably because when an entire country / empire is completely defeated and wiped out forever there are no records left to indicate its existance. If there were records, then it wasn't completely wiped out.

The Arawak Indians located on the island Hispaniola were wiped out by the Spanish within 25 years of Columbus discovering them there. The French later settled in the western third of the island in present-day Haiti. Being that today the country is 95% black, 5% white, and 0% Indian, I'd say that is proof enough that they were wiped out.
You would have to go to other islands to find surviving descendents of the Arawak thanks to the Spanish.

During their advances, after a conquest the Mongols would literally behead EVERYONE. South Central Asia is the 1300s was largely depopulated with only the artesans being taken away as slaves. The ancient aqueduct system from the Caspian Sea into northern Persia was destroyed because some survivors were found hiding in it, and that part of the world to this day has not completely recovered economically.
