never seen it happen.

I think that you are referring to an advanced forward fall: where the opponent jumps over his own arm in a joint lock. It requires that you master the technique of falling as good as possible. Once you understand the mechanics of how a fall is done it's basically a matter of letting go of your fears and putting a bit of spring in your step and jumping into the forward fall. If you can do a good forward fall you'll never go wrong. I've had a few akward falls on my side but never on my head

As long as the Nage or Thrower does not let go of the Uke or Attacker's arm and controls the momentum of the throw, then the Uke will usually land feet first then on his side. So it's important that an instructor is present to supervise lower rank students when performing throws that result in breakfalls or at least a senior student perform the throw on the lower ranked student so he can practice his breakfall safely. But most beginners and low level students just practice rolling after a throw instead to minimize injury from accidents, so you shouldn't have to worry about falling on your head until you've reached mid level rank.

Aikidoka learn to go with the flow .Once their partner gets them to a certain point they go with the throw to roll out and return to their feet thus regaining some control of the situation.Judo sacrifice and counter throws are based on the same principle and most judo champs are excellent at counter throws .KIYUZO MIFUNE perhaps the greatest judoka to ever live wieghed on his best day 140 pounds but into his 60s he contested with 200 plus pounders and tossed them about like children mainly by counter throws using their force to throw them .Someone said "it was like watching elephants trying to throw a butterfly"

Another reason aikidoka go with the throw is if the lock used to implement the throw was struggled against the result could be dislocated wrists elbows and/or shoulder joints and in 2 cases I know of break the neck or back.
No they never land on their head they are in control at all times even when thrown.

In Aikido you will learn proper break-fall techniques. Yes you will be upside down in the air when you are being thrown but as long as you practice proper form it will not hurt.

Yes, I've seen it happen before. Actually in any martial art that has a heavy dose of thows, there is the potential of "not landing right".

As the thrower, it is your responsibility to make sure your partner is going to land OK.
