Traditionally, I think the terms "lady" and "gentleman" are associated: so that logically, he would be referred to as the "first gentleman": but come to think of it, sounds pretty corny doesn't?

Probably Bill, or Mr. Clinton. Maybe the First Laddie. Former President Bill Clinton is another possiblity.
