Funny ... when I read the question line I was going to say exactly that - Ruthanna sounds like Rosanna with a lisp!
But in the middle spot, I wouldn't fret about it. And if you can honor a proud grandma and please yourselves, then it's a great choice.
Of your first names, I think Moira Ruthann Gray is, by far, the best choice. Bonnie is rather insubstantial; Lily is lovely but terribly popular, and combined with a common surname raises the chances that she'll be one of many Lily Grays on this Earth.
Moira Gray is familiar and classic, but not so often heard that your daughter will be one of many. I'd use Moira Ruthann Gray without hesitation!

I don't personally care for that sort of combo name, in general, and Ruthann(a) tends to sound particularly wordy/lispy, as you mentioned.
If you really prefer longer middle names, I'd instead consider keeping Ruth and Ann/Anna separate or even using another family name with Ruth (Lily Ruth Amelia, Moira Vivian Ruth .... something along those lines). You might also like Ruta -- the Polish and Lithuanian form of Ruth that happens to tie in the A beginning of Ann (a sort of combination of Ruth Ann, if you will). ;)

I agree Ruthann might sound a bit funny ("like Rosanna with a lisp" lol) Maybe hyphenate it to Ruth-ann or Ruth-Anna? that way people will be able to see that it is a composite name. Or even just two middle names ie. Bonnie Ruth Ann Grey.

I think maybe you should just use one ffor one daughter and another for a second daughter, or if you wanted to use both at least seperate them. Perhaps not even use thier names but maybe a name that means the same or starts with the same letter.
With the Anne/Anna thing,i may just be being biased but my middle name is Anna and I love it but I would hate it if it was Anne so i'd go with Anna

I prefer Ruthann over Ruthanna. I am actually really like Ruthann. It has that classical feeling while being a bit updated. I actually think I like it better as a first name LOL

Ruthanna. The name is growing on me already. Of the names on your list, I like Bonnie Ruthanna or Lily Ruthanna.
My personal choice would be Naomi Ruth because of the Bible story of Ruth and Naomi.
