if he didnt care then it would be a nightmare of a job to go to everyday cause he wouldnt like it. Its hard to draw a line between doing something because you care because you get paid big for it and doing something because you care and as a nice "side'effect" you get paid well for it. No one can know of this except someone who knows his intentions and heart.

Whether Dr. Phil conducts his therapy in public ... with an audience, or in the privacy of his office ... makes no difference. The reason he's in the field he's in, is because he does care. I mean really, would any of us listen to people cry, whine and have explosive episodes of anger ... for money, and money alone? I know I wouldn't. He has to care, or he wouldn't put up with such a stressful occupation. :)

Some yes. I guess the ones that really touch him. The others I think he could care less because they don't seem to care about their own situation. Plus he is getting paid.

I think he cares about his EGO and his increasing paycheck and selling books.
I think the whole thing with his wife being in the audience and walking out together is WEIRD, implies lack of trust, and in general is ridiculous. Also ridiculous is his son trying to emulate his dad without experience, and his wife writing books...It's all about the $$$. His daughter-in-law being one of those Playboy triplets is also kind of weird...but, to each their own.
