If you are divorced, you provide a copy of the decree to the Social Security administration, and get a new card with your old name back. Then you can change everything else. Shouldn't cost you anything, or a nominal fee at most.

In Georgia, if you request it during the divorce proceedings, it's free. My ex wanted it so "when I get famous, people from high school will know who I am". Since then, she's tried using her old married name (MY last name) to fill out credit card applications several times. Thank goodness for Equifax.

If it is because of divorce it won't cost you anything. All you have to do is change it at the social security office, DMV, and anywhere else your signature is required (bank accounts, credit cards)Doctor's office, insurance paperwork, tax forms etc.

If you are divorced contact social security and request the form and once you get the new social security card you go to dmv-you only need to show that you got a divorce and the new social security card-its not that much to replace your license.

Take your divorce decree to the DMV and social security office and use it like you used your marriage license to change it the first time.

I wouldnt suggest using it again like the second answerer said. That is illegal. Should have handled it in the divorce. Might cost a little to do it now.

If the divorce is not final, have it written into the divorce decree.

If it is "final" you could see if it's cheaper to just change it, or to put an "addendum" on the decree. You'd need to check with a lawyer to see if your state does an "addendum" or not.
