Go with the "easy" stuff first. Not to imply that the experience was in any way "easy" but some questions are easier to deal with and you can "warm up" an interview with them.
"Easy" questions are those that don't require a lot of thought on the part of the interviewee such as name (make sure you have it spelled right and you can pronounce it correctly), country of origin, date of birth, etc.
Ok, now we "graduate" to the more difficult ones.
What camp were you in?
How did you come to be there?
Was your family there with you?
How many of your family survived?
What were the camp conditions like?
Did any of the camp guards show any compassion?
Who liberated the camp?
What was that experience like?
What did you do later?
In general, you want to avoid yes or no questions (those are known as closed end questions). Don't be afraid to ask questions that are not on your list! Many times in an interview the person will say something you wish to follow up on.
Most likely the person that volunteered for this has done many of these, so you should get along fine ... but he or she may become extremely emotional.
Finally, before you go into the interview, educate yourself! Your interviewee will appreciate that effort and it will help you understand what he or she is talking about. A good place to do that is the U.S. Holocaust Museum http://www.ushmm.org/

I would ask were you scared not knowing whats going to happen to you.
Did you think about God alot?
Did you laugh or smile with others?
Did you tell stories to keep your minds occupied?
How were the soldiers keeping you in the camps treating you?
Were there any soldiers with kind hearts?
Or were they all cold and mean?
how was it before the holocaust?
did anyone have health problems in the camps before coming there?
How did the adults give the children hope?
