The last name goes in the middle, but which name goes first? The husband's or the wife's name? For instance, Dick and Jane Smith; Would it be DSJ of JSD?

So your example would be JSD

I have seen it both ways also. Depends on the occasion or reason to have them done or who its for you or him.

Ive seen both... I prefer the girls name going first....
