Does anyone know this game??

Question

I used to play it at school about 10 years ago, and I remember it had all of these geometric shapes, and you would make this worm-like shape go along all of the geometric shapes?? You could also choose different backgrounds and music to go with it?? I know this is a hard one to figure out, so if you have even the slightest clue of what it might be, please answer!!
