I need to file a Form 1040X to get a refund of my taxes from 2006 but the form refers to questions in the booklet which I do not have. Any help would be greatly appreciated.

Go to irs.gov, then to forms and publications (on the left), then to form and instruction number, then scroll down to 1040X instructions.

The easiest way to do a 1040X is to fill out another corrected return, then lay that side by side with the one you originally filed.

you need to file a form 4868 first
