Pilate's exercise, build the core strength, which will support the back and then once that is strong, you can build up other parts. I have Rheumatoid Arthritis, Prolapsed Disc find Pilate's excellent.

if you mean big stomach as in obese, then you should try and lose some weight. The extra weight must be putting the strain effect on the back.

If you cannot exercise then your primary method for weight loss will be to cut calories. Talk to your doctor about what exercise you can safely do to not cause further issue with your back.
