It doesn't matter how you jump or run or skip, the amount of exercise determines your cardiovascular training.
The different ways to skip rope helps train your muscles to move. The reason boxers do other things than just skip in the same place is to work on their movement and timing, it is not related to their hearts.

Skipping rope helps your heart by taxing your cardiovascular system, there's no harm in it. To improve your footwork you should be moving all over the place instead of staying in one spot. In fact, Ali WOULD be moving all over the place when he'd skip and not just stay in one spot.

it is not bad for the heart.Skip any way you want.
Skipping is purely for conditoningAnd it is does not teach or footwork directly.
The idea of mixing it up,moving back and sideways etc is just to get different muscles in the legs and ankles working so you can get all round fitter and 'lighter' legs.
This way your footwork stays light for longer in a fight.

ALL KIND OF SKIPPING IS GOOD FOR YOUR HEART, SKIPPING BACK AND FORTH IS MORE HARD FOR THE HEART BUT THAT WILL MAKE YOUR HEART STRONGER
