Proof it. If you get foaming activity, then it's still good. My guess is that you will have enough live yeast to get a good rise in your bread after only 3 months.

It's a toss up- sometimes it's still okay, and sometimes it will wreck a recipe.

no, buy new yeast...an expired yeast is dead.
