It means the ingredient hasn't been preserved. As an example, a fresh peach that still has the skin and the pit instead of a canned or frozen peach that has been prepared and preserved. Another example would be fresh parsley instead of dried parsley. For fresh herbs, one would use 3 times as much as the same herb that's been dried...the drying/preserving process for herbs concentrates their flavor.

Fresh means not frozen or canned, IN SOME CASES!!! Fresh crab meat frequently comes in cans that need to be refrigerated. Fresh sushi grade fish is frozen first (to kill bacteria and parasites) and then thawed.
