I am a former chef and all of the people are right, the last fellow with the same name as mine is more correct.
Years ago and probably when your Grandmother was drinking it that was one way of getting it, now a days a Dave C sald it is milk with milk solids, a bacterial culture similar to yougart and thickeners sometimes.
As for uses other than drinking it, I like it with a bit og sugar and a squeeze of lemon, I learned this from Danish Chef I worked for, but it is primarilay a cooking/baking ingredient, either in pancakes, biscuits, cakes, salad dressings and sauces.
I make ranch dressing with it, buttermilk biscuits, a sour cream lemon cake, buttermilk and buckwheat pancakes and it is good for thinning chip dip and other dishes with out changing the taste.

Many, many years ago buttermilk was the the left over milky liquid after butter was made. The milk was allowed to 'ferment' to develop a thicker body and tangy flavor to either be drank or used for cooking. Today, buttermilk is made in dairies typically using lowfat milk, skim powder and adding a bacteria culture similar to the type used for making sour creams, yougurts or cheeses and then allowing the mixture to incubate until the right flavor (sour) and body is developed. Not many companies know how to make a 'good' buttermilk that is drinkable anymore and, like scotch, you have to acquire a taste for it. Most buttermilk today is used in baking such as biscuits, pancakes, and the like.

buttermilk is milk that has been soured by using bacteria. Sort of like yogurt, but a different type of bacteria.
What's it used for?
Some people drink it (I've noticed mainly old people drink buttermilk), but for the most part people use it to give baked goods (waffles, pancakes, biscuits, quick breads)a little tanginess. Also, buttermilk can be used to marinade chicken for deep frying.

It's as Dave said. Milk soured by a culture.
It's used as the acid ingredient in quick breads. Irish soda bread gets it's rise from butter milk and baking soda. The same for buttermilk pancakes or scones.

I believe that was only condemned by the little known and later edited out 11th commandment, 'thou shalt not indulge in fermented dairy products'. So yes, eat buttermilk and it's eternal suffering for you.

Buttermilk is the part of the milk that is left over from processing cream into butter.
It's used in cooking.... Muffins, pancakes, waffles, breads, cakes, I even do a potato salad with it.
you can also sour regular milk by putting a spoonful of white vinegar, you know it's "right" when it sort of starts curdle.
