You would definitely need more radiation shielding if you wanted to go up into the Van Allen belt.

There are different opinions. Most people agree that once the mass-to-payload ratio goes beyond a certain point, multi-stage rockets have an advantage.

Higher orbits require a great deal more energy per pound of payload, hence they tend to have a much higher mass (including fuel) to payload ratio, and hence tend to be multi-stage.

For lower orbits, less energy is required, so single stage rockets make sense.
