i dreamt i was carrying my baby niece in one of those baby back packs where the legs stickout, and all ican remember next is that i swapped her out for an old lady who i remember to be my grama. i then put my grama in the baby back pack and haul her around for a while. then i wake up.

There is nothing wrong with considering the needs of others, but it sounds like you are carrying burdens that belong to others, not you......... Helping others is a good thing, but don't let your own needs or your own life get lost along the way...........

Your sister (or brother) is going to need your help, and you're going to lend a hand. The baby on your back is symbolic of taking the burden weight of caring, and looking out for her welfare. Later on, its your grandmother who's going to need help, and again, you will take the burden weight of caring, and looking out for her welfare.
