It's execution day. The prisoner assaults or murders a visitor, witness, or guard. Do they go ahead with the scheduled execution? Do they delay the execution in order to prosecute the most recent crime? Or are there enough safeguards in place to prevent such a thing from ever happening?

Where? in California the defense lawyer would tie up courts another ten years, Florida might delay a couple hours to add charge /info to paper work,continue proceedings. Texas the alert, capable guards would stop the attack by stopping condemned permanently- then adjust paperwork to note early time of death, manner of death, call out NEXT. Most places serious about executions get ready and raise security level the day or so before scheduled execution- one concern is suicide attempts. Condemned usually somewhat restrained- belt chains, cuffs, leg shackles if worries about disruptions. The use of tranquilizers in last meal is a known process.
