I did the same thing...then I went to this recipe...
1 can sweentened condensed milk
1 pkg. chocolate chips
1 teaspoon vanilla
Mix chips and milk together, over low heat until all melted stirring constantly.
add vanilla...pour into foil lined pan, cool 2 hours..remove fudge from refridge...remove foil...cut into pieces..you can add nuts if you want...
I have made this 4 times in the last couple of days and it has not failed me once..good luck...

It's probably a temperature problem. If you live at a high altitude, the ingredients will start to boil at a lower temperature than the recipe wants. Get a candy thermometer and adjust it for your altitude.
