If you're thinking about it. You're not crazy. People who are mentally unstable usually are not aware that they are so.
I wouldn't suggest on-line quizzes, they are generic and geared to specific criteria. if you're really worried, consider speaking to someone...even if it's over a 24 hr phone line.

Well, "lost it" is a pretty vague term, so answering can be tricky. And then there's the question of why you don't want to see a M.D. If you want an evaluation for mental illness, there are mental health professionals other than medical doctors (Therapists, Psychologists, Social workers) who may be able to help you. Licensing requirements vary from state to state, so check the credentials of whomever you see carefully.
If you are looking for an online "self-test" to decide whether or not you should seek further help, the US national institute of Mental Health (NIMH) has information and quizzes for many common conditions that you can look at (link below)

You start to isolate and find it nearly impossible to get out of bed or want to. You may sleep all day to avoid life.
You give up showering and walk around wearing the same clothes.
You have no desire to eat or at the extreme may overeat (usually junk food)
Your place looks like a total mess, because you really don't care.
You lose your sense of concentration and even find it hard to keep up with a tv show.
You think of a way out and dying
You are just feeling unable to cope with everything and anything that comes your way.
These are some of the clues to a mental breakdown.
I've witnessed a family member go through this. The longer you ignore it, the greater chance of winding up in the hospital. There is treatment though.

The person will not seem "connected" to the world around them. They may cry for unknown reasons. May have a spaced-out look. Will make inappropriate comments.
They may remain bed-bound, lose an interest in everything including: sex, appearance, food, friends and family. They may become paranoid and see that people trying to help them are in some way trying to trick and harm them.
They can't concentrate or do work.
Their memory and though processes become impaired.
They think of death often

I think others close to him/her will be able to provide the feedback. There are different mental disorders that can cause a person to "lose it" . Depression is the biggest one. There are self tests for depression.

If it's someone else, it may be the way they look, (unkept, ratty, soiled clothes...) or the way they talk or interact.
If you are worried about yourself, just go to a doctor. that is the only way to be sure.
