Brake pads are equipped with a noise making piece of metal to let you know it is time for new brake pads. I would guess if this just started you might have a few hundred miles left on them.
If you are asking this question, I would guess that you do not know how to change them yourself. It is fairly cheap- expect to pay around $200-ish, depending on who you find to do it for you. Unless you know a good friend that you trust totally with your life, take your van to someone bonded for this type of work. Your life (and your family) likely depends on this simple job being done correctly.

A loud metal to metal noise from the brakes means that there is a problem with the brake pads. Either the pad broke apart, or it has worn away. In either case you need to have the brakes repaired ASAP. Do not drive the car anywhere other than to the shop!!
Most likely, if you have been driving the van at all, you are going to need to replace the rotor, as well as the brake pads!
The front pads on the Caravan will wear out early, if the rear brakes are out of adjustment. You need to set the parking brake regularly to have the rear self adjusters work! Otherwise you will need to have the brakes adjusted manually at every oil change!!

The type of noise u r geting from the brake while applying and also feeling vibration on pedal indicates ur brake pads have totally gone and it starts eating up the brake rotors too. if u dont look in to this immeadietly it may lead to end up with the replacement of brake rotors and along with the pads, which will be expensive. try to do the pads replacement as soon as possible and check the rotor run out, if it is with in the service limit, skim the rotor and save the cost for new one.

Most likely it is time for a brake job. There are small metal tabs that rub on the disc when you have reached the point when the pads should be replaced. You may get by with cutting the rotors and reusing them. These need to be measured and maybe turned.

Apparently all these people answering live in warm climates. That scraping sound is snow packed in wheel well. Wait until a thaw before replacing pads. ....you most likely are ok

Sounds like you need new brake pads. If its metal on metal you need them ASAP and you may need to have the rotors turned.

Brake pads getting worn warning it all. Need to replace the pads is all and don't forget to turn the rotators while you are at it.
That is also a good time to repack wheel bearings as well.
Good idea to check rear brake shoes while you are at it if it has rear shoes instead of disc brakes.

It could be your warning tabs on the pads, or some dirt. It might not seem like dirt can do all that, but you would be suprised... I would also check to see if there is a rock or twig stuck in there, that could be the vibration you're feeling.
