My Mother has dementia and may possibly need to move in with me because I am concerned for her safety at times. If, down the road, I can no longer take care of her and need to place her in a nursing home, how can I keep the state from getting her assets if she needs to go on Medicaid to help offset some of the nursing home charges?

Pay for the nursing home.

First, while she's with you, charge her room and board. The longer she lives with you the further her money will go. When a patient goes on Medicaid, the options and care level go down proportionately.
Consult an attorney about power of attorney.
she will go thru most of her assets before she qualifies for Medicaid, with only her monthly income like SS and pension paying her share of her bills.

Change the assets to your name. But you have to do it 3 years before you put her in the home.
