First, you can't disbar a doctor, since they are not members of the bar. That's a lawyer thing.

If you think the doctor has committed errors in judgement and you want them brought before the Florida medical board for a hearing, you can file a complaint with the Florida Board of Medicine at http://www.stateofflorida.com/Portal/DesktopDefault.aspx?tabid=115

However, you might do a lot better just talking to the doctor and asking how to get a fitness for duty certification. Ask what you need to do - diet? exercise? medicine?

It sounds like you have 2 things going on. We're you released from your w/c injury to full duty and then had a medical condition that put you on medical leave?

If you are disabled due to the w/c injury your company is still liable for you. If it is an unrelated medical condition call your employer and request to be seen by the physician to discuss your fitness for duty.

If your physician gave you a rating the insurance company owes you a settlement (money) for the rating.

Hope this helps.

Contact your workman's comp insurance provider and file a complaint and report her to the board of medical review. You will need a copy of your records - she must release them to you by law and you have a right to a copy of them so you can take them to a Dr of your choosing for review. You will have to pay for the dispute but if you were wrongfully terminated based on a fraudulent Dr you may be reimbursed as well as get your job back. Good Luck!
