The penis will start to lose rigidity and girth; so yes, the condom may become loose and slip off/break from the friction. A man should wihdraw after ejaculation, holding the base of the condom, and should not have futher penis-to-vagina contact - some sperm still exists in the penis as it becomes flaccid, so touching or re-insertion can result in pregnancy.
