It is around $15 per month. (Higher if you have sales tax to pay, lower if you use the 3-month recurring credit card option.)
You can pay via credit card, debit card, or prepaid game card (which you can purchase at a store that sells the game).
You have to create an account and password, which you use to manage your account data and to log into the game. Once you log in, you create your character with whatever name is available (not the same as your account name). You can have up to 50 different characters.

No Phone payment option though.
