what this form do and what is it for? do i have to file this information to the annual tax income during april? even i only make about 1000-2000 dollar a year, on what amount that i must file this information for the annual income tax?

Form W-9 is the IRS form used by a company to request your taxpayer identification number. Most often, Form W-9 is sent to independent contractors, consultants, and other self-employed workers. Fill it out by giving your name and Social Security Number, or the name and Employer Identification Number of your business.

By submitting a W-9, you are certifying that the tax id number you are providing is correct and accurate and that you are not subject to backup withholding.

I would add that the information on the W-9 is needed in order to send you a 1099. A company who pays a contractor at least $600 over the course of the year is required to send a 1099 to the contractor and a copy to the IRS.

If you don't give a payer the form, they are supposed to withhold 28% of your payment for taxes and send it the IRS for you. Then the only way to get the money back is to file a tax return.
