As a minimum, they'd come in as a Captain (O-3). That said, they might come in at a higher rank depending on post-graduate qualifications and experience. Best person to talk to is an Army Medical Department (AMEDD) recruiting officer.

Doctors of medicine including a Clinical Psychologist would normally be given the rank of Captain (silver railroad track which are 2 1st Lt's insignia of rank) for the Army, Air Force, and for the Naval Dept they would be called a Sr. or full Lt with the same insignia.

Anywhere from a E-1 to a 0-3.
Just because you have a degree, does not mean you will become an officer.
There are plenty of enlisted members of the military with BS, Masters and PHD's degrees.
Now, if the doctor applied to the military to enter service as a doctor, and passed all the interviews and back ground checks.
And the military needed thier speciality.
Then they would enter service as an 0-3 after the professional officers version of OCS/OTS.
And depending on the speciality, they might recieve special professional pay in addition to thier salary.

With the schooling required to be a Doctor, their rank would have to be at least Captain.

depending on Branch, Component(Active or Reserves) and real world experience, as well as degrees held.. as high as O3 for a Direct Commission.
That's Lieutenant in the Navy and Coast Guard, or Captain everybody else.

In the Navy it's a commander or captain depending on your level of expertise (looking over current interim pay charts)
When looking into the military though, you really have to ask yourself how much personal freedom do you want to give away? I don't want to temper a dream, but when a combat doc is asked does this guy live, or does the unit face dying, what do you do? I could make the call, then again I've been called cold callus, conceited, arrogant, and all the like... I'm not even a doc.

depends on how they were comissioned. 1st LT or even a Capt. But this all depends on if the military accepts psychology because usually its more leaning towards Law, Dentists, Nurses and medical doctors as in the curing of a wound that is seen (i.e. Bullet wound) 1st LT best bet but may go as high as capt. Also depends on what branch you want to join. Army is looking for ANYONE and AF is also big on this.

I don't think a psychologist comes in at the same grade as a MD. MDs come in as captains. I think, but am not sure, that psychologists come in as second lieutenants and belong to the Medical Service Corps and not the Medical Corps.
I know that I've seen lieutenants on the Combat Stress Teams. I don't know if they had doctorates though.
