It could be mange or a food allergy. Usually with mange you will see a red pustual that follows by hair lose.
With a food allergy they will scratch everywhere. This is usually due to an allergic reaction to wheat in the lower ranked pet foods found at walmart or the grocery stores.
Regardless of which one it is, you should take him to the vet.

I would not know what the exact problem was, but I would strogly advise taking your dog to a vet. If you take advice from this site, it may be wrong, and you may end up harming your dog. Your dog may also be in discomfort at the moment, and if you want that to stop, you need professional advice

Bichon's are known for sensitive skin. Try a dog food for sensitive systems and no extra treats. Have a groomer clip his nails and give a medicated bath.

Treat for fleas anyway. Use something GOOD. If that does not stop it, you need to see a vet.

It sounds like mange... this is very contagious. It could have easily been spread from another dog. If left untreated, it can get bad enough to where the puppy has to be put to sleep. Please see your vet.
