You can e-mail, telephone or write to your insurance company or agent. Be sure to have on hand the policy number, type and term of the policy, what the old address was, and what you want it changed to.
They should ask you for identifying facts - Name, age, birth date, etc or may send you a conformation letter/e-mail, just to ensure that you are the one changing the policy and not some one else.
If its other than a life insurance policy, your premiums might change, either up or down, depending on the risk factors of your new location.

As others have mentioned above. Call your insurance agent, or the insurance company directly. It takes effect immediately. They will mail you a new insurance card with your new address. Legally, you have 30 days from the time you move to update your address.
