Yes in the USA you have the right to have a copy of your record some hospitals may request a fee if you are not able to pay they still have to furnish you with a copy It's best to contact the records department of the hospital because they may have a standard form that you have to fill out or you may just have to submit a letter either way it has to be done in writing they may also ask for a copy of your ID
Make sure to state that you want the entire record or else they may just give you parts of your records and you'll have to make a new request if you are denied all or parts of your records ask for the reasons to be put in writing
also each city/state has a time limit on how long they are required to keep your records after the last contact you had with the hospital as a patient. In nyc it's 5 years

It the USA, yes, you do have that right. HIPAA legislation assured it.

It is the case, however, that psych records do have a special exclusion. If the provider believes that it may present a significant detriment to you to review certain parts of the file, he or she can (legally and ethically) withhold those sections.

You have the right to a copy of your records you just have to fill out a form or two and wait until they can run you a copy.I have done this before.

Yes, but it won't be easy.
They will insist that they have to send them to a doctor and the majority of people stop asking at this point.
It helps once you start mentioning legal rights and seeing an attorney.

... sorry to tell u this but theres no way a hospital can release medical records with out a Doctors request form... some times ur not even allowed to go pick them up ur self they mail them directly to the Dr's office or fax them over.... eventough its YOUR medical record they are not uthorized to do such thing

Yes you can have copies of any records from any Dr.s they are yours.
