If you upgrade it will not remove any old programs, but Vista doesn't like older programs and some of them may not run, Also Vista has a lot of compatibility issues with older printers. I recommend you stay with XP for now go to Microsoft.com and see if you can find a fix for the media center problem. Also make sure you run Windows update at least once a month even if it's set for auto update because the auto update doesn't get every update.
Do some maintenance on your machine run a virus scan, spy ware scan, disk clean up, and then do a defrag may not help but won't hurt.

The errors would most likely be fixed when you upgraded to Vista Home Premium.

IF you upgrade the machine, then your other programs should still be there.

IF you do a clean install, then all your old programs will be gone. A clean install first reformats the Hard Drive.

Bear in mind, Vista does not support 16 bit programs. Therefore if any of your older programs are 16 bit, then they will no longer work.

I would never upgrade an OS but always reinstall clean. And that loses all your files unless backed up, and uninstalls all your programs. Many of your older programs may not even work on Vista. Try a reinstallation of MCE instead, either a repair or by the same wipe clean and install method.
