The first is the normal (linear) operating region of the transistor.

The second is a transistor in the "cutoff" switch mode (no collector current, collector voltage at max, or Vcc).

The third occurs when the transistor is "saturated" (fully turned on in the switch mode, collector voltage very close to the emitter voltage).

The last does not normally occur.
