what does that mean & whats a ff86 form?

The would be the SF (Standard Form) 86, which is the base document for the background check for granting or renewing a security clearance.

The applicant puts a ton of personal information into this form (and all its addenda) which the investigator then uses to determine whether or not to recommend granting the applicant a clearance.

it has to do with a Clearance..he's either getting one or it's time for his 5 year review of the one he currently has.
perfectly normal.

it has to do with his EPSQ
have him contact his S-2 if in the army or security clearance people in his unit they will fix it
