Fill a pot with Hot water and let it come to a boil. Put the chocolate in a metal bowl and place over boiling water in pot. Stir it until it is all melted and has a fluid consistency.

How are you melting it? Use a double boiler. Or place the chocolate in a glass bowl placed over top a boiling pot of water. If you don't use a secondary method of heating it you will likely burn it. Often chocolate that has a lot of wax in it - like hershey kisses will only soften but won't melt as good as purer chocolate.
Also too much humidity or water - even on you utensils will cause chocolate to become stiff and grainy. Also make sure that you stir frequently

The heat is to high(not direct,i hope)&you are getting some moistures into it causing it to seize(go crumbly)
Best to use cooking chocolate
HERE'S A SECRET:
if chocolate seizes,rescue it by adding into it a spoon full of vegetable oil&whisk like mad(turns back to chocolate)
TO MELT CHOCOLATE:put choc in a beat proof bowl,place a small saucepan 1/3-1/4full with water&bring to simmer.Place bowl with choc over the top&stir while the steam melts the choc.Keep heat low.Do not let any water touch or enter the bowl(i lift&wipe bottom,every so often)
Depending on what you need choc for,you can add a little cream/butter to help melt(ganache)
Try again,you should be fine(if all else fails..melt in microwave 30-40sec bursts on MED,stir between zaps)

Sounds like your chocolate is ceasing up. Are you melting it in a double boiler? It is the best way. Or, you can place it in a metal bowl over a pan of hot boiling water and let sit until melted. You must use it pretty fast or it will cool off again.

You need to melt chocolate in a heat-proof bowl in a saucepan with water in it. Set the small bowl containing the chocolate in the saucepan with about an inch or two of water in it on low heat. It will melt as the water heats up. Otherwise, it could burn.

You have scorched the choc. it will not melt, its garbage, you'll need to start again, try a double boiler or use the mircro but you must check the choc. every 30 seconds to prevent this from happening again.

if its crumpling you have heated it too much. Either use a double boiler or put in microwave for 20 sec, take out stir, put back in for 5 secs at a time until desired consistency.

What type of chocolate are you trying to melt? how are you trying to melt it? Do you have a candy thermometer? Did you add butter to it, or are you trying to melt it standalone?

Are you using a double boiler? Don't put it directly in a pot on the stove - if you don't have a double boiler then it's perfectly fine to microwave it.
