It should be the man. It is because he knows the best way to put it on (it's his tool) and the women might have long fingernails that could puncture the rubber (unless she planned it from the beginning...).

I think it would be better if the man put the condom on, the woman has no appendage that I can think of that would do any good for her to put it on.

The person who is holding the condom puts it on (the man's penis). It doesn't matter who does it, as long as it's done correctly.
