You better take her to vet then, she'll become dehydrated fast with vomiting and diarrhoea. She's obviously ill, it doesn't really matter how it happened at this stage, you need to get her treated. Maybe she did eat something, maybe she picked up a bacterial infection or a virus not covered by the vaccines - just go to a vet.

have you changed her food ? also you need to puppy proof your house if "Maybe she got intoxicated eating something off the floor" this is a statement that you can make, I would go to the vet if you dont know, not asking online.

Take her to the vet ASAP. She could have gotten sick from the shot itself.
Same symptoms occured with one of my dogs, ended up being distemper.

3 months is very young , so take her to the vet. she can get dehydrated quickly and this can be fatal. And since she is young and her immune system is not fully developed, she can catch viruses and diseases quiet easily so better safe than sorry.

check with the vet -perhaps your dog ate some kind of plant in the house -like pointsettias, they will create these symptoms you see in your dog.
