Are you saying you have all the paperwork etc but just never went and had the visa stamp placed in your passport in your home country? Do you still work for the US company that sponsored the H1?

On your H-1B Approval Notice, it states the duration of the validity for the Approval. Approval Notice is NOT a visa. You are legal on H1B approval, but to enter US again when you travel outside of US, you need to apply and get a visa stamped at the US Embassy or Consulate, within the duration of approval validity.

your h1b status is valid as long as you are still working for the sponsored employer and you does not violate your h1 status. H1 stamping on your passport is required to re-enter to US after traveling aboard. you can apply for h1 stamping as long as it is still valid. you can apply for the h1b stamping at the US embassy/consulate OUTSIDE US. applicants usually apply the visa at the US embassy in their home country.
to apply for the h1 nonimmigrant visa, you will need form DS-156, form DS-157 (for males between ages 16 and 55 years old), your passport, one 2x2 photo, original Form I-797 notice of approval, and supporting documentations such as updated employment letter with information of your salary and position, recent pay stubs (to prove your h1 status in US), W-2 or recent tax return (if applicable),original degree certificates/diplomas, copy of the complete I-129 petition, and Labor Condition Application. please visit the US embassy website of the country you will apply your visa for application fee and appointment. those information is usually located at "non-immigrant visa" and "how to apply for visa". good luck.
