I noticed that they are the same game, but have different publishers. Are there any differences in the game itself?

No difference.

Sega originally published Medieval: Total War (and EA published Shogun: Total War). Activision and Sega co-published Rome:Total War. The box actually have both logos on it.

I am not sure of a difference. However, I am having trouble installing the total realism mod on the SEGA version whereas I never had trouble with the older Activision publisher.
