Traditionally, men dressed to the left. This is why the zipper covering tends to lean towards the left. At least traditionally. These days pants are so baggy that it doesn't make that much of a difference as most guys don't where their pants up that high any more. Personally, I try to find pants that have the smaller waist to hip ratio so I don't feel like the crotch of my pants are halfway down my legs. Consequently, my bulge tends to be rather obvious. Of course, I usually don't hear or see anyone complaining :) So to answer your question, it was traditionally to the left. But I would assume that whatever is more anatomically comfortable for the individual is what is more important.

Straight down if the underwear allows it. Otherwise my equipment tends to default to the left on its own.

Straight down so as that no bends form in the shaft. wear no brief or underware, and just let hang naturaly low and free.
