carers and nurses ALWAYS appreciate chocolate.
If you want to give something a bit different - make up a platter of nice sandwiches, cakes, etc for them to share out in the office.
If it's for one specific carer then what about a bottle of wine?

We have 3 wonderful regular caregivers that take care of a family member at a nursing home. Last year we gave them each a card, and inside put a little thank you money, a few scratch off lottery tickets, and a gift certificate for a manicure/ pedicure. They ended up going together to the spa appointment, and then went out for happy hour and played the lottery tickets. They don't get pampered very often, and it gave them a chance to get out and have some fun. We will do the same this year.

These guys have given you some great answers. I can't think of anything that would be much different than their answers. Just remember this, a lot of those residents are lonely (not because of the care they receive, but because of the lack of family support), and anything you do for them will be greatly appreciated. Many of them will adore you for the attention they will get. Dont forget you can also make homemade cards and gift tags for the items you take to them. Another idea that you might try, that wouldnt cost you anything, and would make them just as happy. Contact a local elementary school, contact a nursing home, get the kids in the school to adopt-a-grandparent for the holiday. Anything to do with kids always brightens their day. The kids could send cards, letters, gifts, or even a field day to visit with them. It's not really about the gifts they receive, most of them care more about the attention. Any way you can make them smile will be worth more than the most expensive present you can get them.

I recently left my job at a nursing home. The residents (a.k.a. "patients") would give me things like pens simply because they were in the nursing home and they really didn't get out much. It was the nicest thing ever! One woman told me that she was giving me the pen because I was nice to her and she liked giving people gifts who were nice to her. Any little thing is really wonderful to receive from a resident. I'm not just saying that to make myself seem really caring. I honestly loved all my residents and I kept everything they ever gave me. I use the pen all the time and it reminds me of her. I can say from experience that a caregiver would be well pleased to receive anything from a resident no matter what the cost or item!

a stethoscope would be a nice gift (dont have to be the expensive ones thou) but if u cant find a cheap but quality one, u can always just buy an accessory for it (dangling/hanging coala or whatever that can hang in a steth)

FOOD! bake brownies and other goodies and take them in a cute basket. Take it to them at work on Christmas. They will LOVE it! You might also want to give them a pin of an angel wearing a nurses hat. anything small that won't break your budget but will give them a smile and let them know they are appreciated!!

For several years I have sent a big fruit basket to my vet's office. They always comment what a nice change it is from candy and sweets.

my mom was in a nursing home. and my dad always left a box of donuts in the room. and for christmas, sometimes he would give them small trinkets.
