It usually means "free of societies restrictions" in one way or another. Historically it was a perhaps nicer way to describe someone who had sex outside of marriage, or refused to attend church, or took odd political positions like advocating voting for women, or something radical along those lines: something that violated the accepted limits of polite behavior in a society.
