or just say to her that i am not sure if i could give her a card. and tell her that i appreciate everything i do for her and say that i hope she has a great holiday.

I would just tell her that you appreciate everything she does for you and wish her Merry Christmas. Some therapists are really touchy about receiving anything from a client besides their hourly fee, so you're safe and still polite if you just give her best wishes in person.

I too think it is appropriate just to shake their hand at your last session before Christmas, thank them for their help and wish them a Happy Christmas. Anything else may be a little too personal.
