What does the following statement mean in simple words:

During the past 5 years, have you or any family member accompanying you lived in any other country than your country of citizenship or permanent residence for more than 6 months?

I am going to make it easier. Substitute parts of the sentence with your own information.

During the past 5 years, have
you or any family member accompanying you
lived in any other country than
your country of citizenship or permanent residence
for more than 6 months?

Replace "you or any family member accompanying you" with the names for your immediate family, the people making the application with you. If you are travelling with your wife and child put their names in there.

Do you have permanent residence in a country that is not your origin or citizenship?
If not, your citizenship is what is valid. So put your country of citizenship in where it says "your country of citizenship or permanent residence", and if you do have permanent residency elsewhere, add that.

My example if I was applying to live in Australia with my wife:

I or my wife
lived in any other country than
USA or UK
for more than 6 months?

Much easier once you take out the "variables" and replace them with actual facts.

Let's say you are a citizen of Canada.

Since the year 2002, have you lived in any country besides Canada for more than 6 months?

Since the year 2002, has any member of your family (not your whole family, just the people coming to America with you) lived in any country besides Canada for more than 6 months?

Have you lived anywhere else than your birth country for more
than 6 months? (in the past 5 years)
That also means anyone else in the family that was with you.

Within the last five years, have you or any other person that you're coming into X country with -- lived in another country, besides your country of origin or the country in which is your permanent residence -- for more than six months.

Let's say Mr. & Mrs. Chang and their two children are from China. They are applying to live in the U.K. They have always lived in China and have never lived elsewhere. They will answer "No" to the above question.

Mr. & Mrs. Petal are from India. They are applying to move to the U.S. Mr. Petal studied at University in the U.K. for a year in 2003. Mr. Petal will answer "Yes" to the following question (and perhaps specify the U.K., what year he lived in the U.K.)
