Need to apply for an SS card at your local Social Security office, in person. Birth certificates can be obtained from the City Clerk of the town that you were born in. I think you'll need to get the birth certificate first in order to have it as identification to get your SS card.

You can get a birth certificate and passport at the register of deeds for your county. You will need to go to your local social security building or go on the website and request a duplicate card. You will need two other forms of identification.

You'll need to go the the office of vital statistics in your area and fill out an application then they can get you copies. It will cost you between $30 and $50 though and can take 6 to 8 weeks, so if you considering traveling, you'd probably better get started soon.

You can order a copy of your birth certificate on line from many jurisdictions. Start here for who to contact in your state: http://www.cdc.gov/nchs/howto/w2w/w2welcom.htm
To get a copy of your SS card, start here: http://ssa-custhelp.ssa.gov/cgi-bin/ssa.cfg/php/enduser/std_adp.php?p_faqid=251
