A DoctOr's note is usually, an A5 pre-printed document, that your doctor will fill in for you (either by hand or with the practice's computer)..

Not to be confused with a self-certification document, which is a folded A4 pre-printed form that you get from your local surgery and fill in yourself

1st. one - Difficult to forge

2nd. one - You don't have to forge

Seriously, though, given that misspelling,
you need to find excuses to stay in school longer,
not find ways to get out of it

You wont be able to make one, you're better off saying that you had a family emergency or something.
