It was obviously not a pregnancy, unless somethink genetically was wrong with the little boy and he was pregnant.
The little boy was trying to tell his mom how much stomache pain he was in. Obviously a melon split wide inside his tummy would feel horrible. On the other hand he could have been describing a situation in a mental sense about how nervous he was during school that day in a stressful situation.
