All of the above are correct except the hobo thing that's just bad taste and rude. Gout however does not only effect the big toe but yes it mostly affects toes. I had/have this and the only time it hit me was the most painful thing I ever felt even the pressure of a light sheet touching it was killing me. Now i talk a Chinese herbal mixture to keep uric acid from building up. I use Chinese herbal remedies for a lot of things instead of drugs because the Chinese have been curing things like this for thousands of years. And they do work.

Another folk remedy is to eat a few cherries or drink black cherry juice daily to avoid gouty flare-ups. I can't vouch for it, but several people have told me that it works.
