I have severe arthritis and back problems and have had several surgeries and will have more in January. My surgeon says that he would rather I smoke than gain weight. Also, hot peppers do offer pain relief. I've been told to eat as many hot peppers each day as I can tolerate. Luckily I love jalepenos, cayenne peppers and tabasco peppers and I eat several with my meals. It does offer pain relief!
As a note: I believe God put everything on this earth that we need and often it's just finding something natural that has health benefits without all the heavy-duty drugs. For example, garlic lowers blood pressure and cinnamon (just a sprinkling a day) keeps glucose levels lower.

The amino acid capsacin is present in peppers which accounts for its pain relieving property.
It is sometimes present in topical pain relievers.
It is usually applied topically and the burning sensation will serve as a counter irritant to relieve pain.

I dont know anything regarding the pain relieving function of nicotine
