Smoking is an FAA violation,,,, and guess what the first letter means... "FEDERAL."
Which mean,, if in any way a flight attendant sees smoke billowing out a lavatory with your friend coming out of it in slomo like a rap video... I'll bet you $500.00 that there'll be cops waiting for him at the end of the flight.
Buy em a Copenhagen and tell him to dip... That'll curb his smoking habit...
I myself is a smoker,,, I work all day sometimes with out a puff of cigarette. I just deal with it.

Tell him to get some patches and/or gum for his flight. All commercial flights crossing the Atlantic are non-smoking. At first they said that some charters would have smoking flights but I've never seen this and it may not have even happened.
My coworkers resorted to patches when non-smoking flights were introduced. I thought it would be a disaster but it actually was fine. The smokers told me that they were okay because they didn't see or smell ciggies the whole time. There were no reminders.
Smoking in the lavs is a bad idea and he could be arrested on arrival, or even sent back to the U.K. the same day.
In the U.S. most airports are non-smoking with strict rules about where you can light up-even outside. Perhaps wait to tell him this!

There are no scheduled airlines that allow smoking on flights that originate in the USA. You might look at some charter flights but those are very risky in terms of not flying and you not getting your money back.
Your friend's choices are to: stay home and smoke or suck on a nicotine lozenge every hour he is in the plane.

Get the patch. Or chew the gum. Flight Crew suffer from that, too! You will be hard pressed to find an Airline that permits smoking on board the Aircraft. Those days are long over!

Smokers make due when they have to. Your telling me that there aren't 8 hours in a day where he isn't smoking a cigarette. He has to sleep sometime.....
Drink a few shots enroute and catch some zzzz's. Then you won't have to stress over how he may or may not react to not being able to smoke.

looks like you'll be going without him. No smoking on British Airways. How sad to deprive yourself of a really cool vacation just because you can't go without a cigarette for a few hours.

Have him get the patch and/or some gum to help with the nicotine cravings. Then have him pick up some smokeless cigarettes (aeros).
Alternatively, you could have him get a sedative before you leave and sleep through the flight.
