Since I don't understand the first part of you so many questions, I will answer the parts that I know based on my personal experience. The main concern for them is that they don't allow foreigners with red flag to enter the US.
They do all the checking to make sure you are telling everything truthfully on the application and the I-20 the school issued is valid from the really existing schools and such. They want to find out that you are intent is true that you only want to go to school while in the US, not to do something else, such as working illegally or to marry someone for just green card . They will make sure your ties to the home country is strong enough that you will come back after the school is done, since F-1 is nonimmigrant visa. Also, to make sure you have sufficient funds to afford school tuition and living expenses for the entire duration of school years.
Visa is more like stamped sticker under lamination on your passport page and on F-1 visa you will see your picture, your name, visa number, the school name and address, how long the visa is valid, where it was issued and US seal.
If you are all truthful, there is nothing to worry about for the interview. Although do not ever lie to the Immigration Officiers and do not joke with them. Usually 2 officers conduct the interview and both of them bi-lingual with English and your language. Good luck!

From this and your other questions, it sounds like you are looking for ways to provide the "right" answers to your clients who are seeking visas to the US. I hope nobody feels the need to assist you in this endeavor. The bottom line is that if visa applicants have a legitimate reason to go to the US, have the financial means to do so, and are able to demonstrate a willingness to obey all of the visa requirements, the likelihood is that a visa will be issued.

Also, if you had a legitimate need to know what a US visa looks like, there are legitimate sources of examples - not the Internet. This all sounds very fishy.
